A registered professional nurse who utilizes the nursing process in providing nursing care and assumes the, responsibility and accountability for prescribing, delegating and coordinating that care. Assumes responsibility for direction of patient care through the resource nurse role; acts as a mentor for staff and students. He/she works collaboratively with unit resources in assessing educational needs and implementing programs for select staff.

Responsibilities

  • 1. Demonstrates advanced skill in assessment of health care needs.
    • 1.1 Demonstrates advanced skill in interviewing, observing and collecting health care data, performing systems assessment, identifying patient support systems on admission according to hospital policy.
  • 2. Demonstrates advanced skill in planning for patient health care needs.
    • 2.1 Communicates the patient's health status through the initiation and maintenance of the clinical pathways and develops new clinical pathways for unit, based on unit census of high risk/high volume case types.
  • 3. Demonstrates advanced skills in implementing care to meet patient care needs.
    • 3.1 Prioritizes patient care needs based on assessments.
  • 4. Demonstrates advanced skill in evaluating care delivered.
    • 4.1 Assesses patient responses to medical and nursing interventions based on expected outcomes.
  • 5. Documents outcomes appropriately, accurately and consistently.
    • 5.1 Completes Admission Assessment Forms.
  • 6. Performs decentralized laboratory testing as credentialed and following established policies and procedures.
    • 6.1 Appropriately collects specimens, performs test and interprets results.
  • 7. Demonstrates advanced leadership skills and professional growth, as evidenced by supervisory feedback, direct supervision and documentation of activities.
    • 7.1 Initiates self-evaluation of nursing practice through the independent development of appropriate goals.
  • 8. This eval must be accompanied by documentation of all the activities performed during the eval year as listed below such as, contact hours, inservice materials, preceptor class attendance, transcripts from credits earned, listing of preceptees, etc.

Qualifications

EDUCATION

 

Graduation from an accredited School of Nursing. BSN or higher degree preferred. If non BSN, must show evidence of active enrollment in a minimum of a BSN program.

 

EXPERIENCE

 

ACLS & PALS REQUIRED/CRITICAL CARE EXPERIENCE REQUIRED

 

LICENSURE

 

Licensed and a currently registered nurse in the State of Connecticut. All newly hired Registered Nurses must have a current BCLS certification or obtain within one month of hire.

 

SPECIAL SKILLS

 

Demonstrated leadership ability, good interpersonal skills.All staff members must meet the criteria for their clinical level on an annual basis, as defined in the Professional Ladder. The individual nurse must submit written documentation to their manager, a checklist will be provided on the intranet under the Shared Governance site (bottom of page under documents), supporting maintenance of advanced clinical status to the Manager. The nurses supporting documentation must be submitted thirty days before annual evaluation is due or by the nurse manager's discretion. The checklist will then be sent to the director for their signature and filed in your personnel folder. Failure to meet criteria by the evaluation due date will result in a demotion and a pay rate decrease. The employee may voluntarily withdraw their clinical ladder status and be demoted. 1.Active member of a hospital-wide committee or Chair a unit based or hospital wide committee in a leadership role, with evidence of participation and attendance of 75% of meetings within 6 calendar months of application. Provide a letter from the Committee chairperson summarizing participation. ** Use template provided. 2. Participates in staff meeting by presenting above committee work. This should be reflected in a summary of staff meeting minutes. Provide a copy of staff meeting minutes. 3. Attend a minimum of 6 in-services. Provide copies of in-service sign in sheets or copy of my transcript tab from health stream from those in-services you have attended.(Continued in Accountability)
